Direct Access to Physical Therapy Services
At OrthoRehab Specialists, we believe that timely evaluation and treatment by a licensed physical therapist can accelerate your recovery and save you both time and money. With direct access, you can be evaluated and treated without first obtaining a physician’s prescription. This approach removes unnecessary delays and empowers you to take control of your rehabilitation journey from the moment you decide to seek help.
What Is Direct Access?
Direct access allows you to consult a licensed physical therapist without first seeing a physician. Under Minnesota law, you may receive up to 90 days of care for a new condition solely under the care of a physical therapist. If you are managing a chronic condition already diagnosed by a physician, you may continue therapy without a new referral. Direct access removes barriers to care and ensures you receive expert evaluation immediately.
Benefits of Direct Access
- Faster Care: Begin your evaluation and treatment on the same day you contact us instead of waiting for an additional medical appointment.
- Cost Savings: Avoid extra co-pays, office fees, and specialist consultations that may not be necessary.
- Improved Outcomes: Early intervention by a physical therapist can reduce the risk of chronic pain and prevent surgical intervention.
- Personalized Treatment: Our therapists design a custom plan from day one based on your history, goals, and functional demands.
Minnesota Direct Access Law
In Minnesota, direct access permits patients to receive physical therapy evaluation and treatment for up to 90 days without a physician referral. A previously diagnosed chronic condition may also be managed without a new prescription. Medicare patients must still have a physician referral, but most private insurance plans honor direct access as long as services are covered. Before your first visit, check your benefits on our Insurance & Payment Options page to confirm coverage.
Who Qualifies for Direct Access?
Direct access is available to anyone with a new musculoskeletal injury or chronic condition, including:
- Ankle sprains, plantar fasciitis, knee pain
- Back and neck pain, sciatica
- Post-operative rehabilitation after joint replacement or repair
- Overuse injuries from sports or repetitive work
- Balance disorders and fall risk in older adults
If you are unsure whether direct access applies to your situation, our administrative team can verify benefits and explain any limitations based on your plan.

What to Expect at Your First Visit
Your initial direct access evaluation will last about 60 minutes and include:
- Patient Interview: Discussion of your health history, symptom onset, aggravating and relieving factors, and functional goals.
- Physical Assessment: Measurements of range of motion, muscle strength, flexibility, posture, and balance.
- Orthopedic Testing: Special tests to evaluate ligament integrity, joint stability, and nerve tension as needed.
- Functional Movement Analysis: Observation of gait, squatting, lifting, and sport-specific motions to identify compensations and asymmetries.
- Goal Setting: Collaborative development of short-term and long-term objectives tailored to your lifestyle.
- Treatment Plan: Outline of manual therapy, therapeutic exercises, functional training, and a personalized home exercise program.
Follow-Up Visits & Progress Tracking
After your evaluation, most patients attend two to three sessions per week depending on condition severity. We reassess progress regularly by tracking:
- Range of motion improvements
- Strength gains using objective measures
- Balance and functional test performance
- Pain reduction on standardized scales
As you achieve milestones, we transition you to a maintenance phase and provide ongoing education to prevent relapse and support long-term health.
Exceptions & Medicare Patients
Direct access does not apply to Medicare beneficiaries. Medicare patients require a physician referral before starting physical therapy. Our team will help coordinate any necessary paperwork and referrals to ensure you can begin care without undue delay.

Frequently Asked Questions
Do I need a physician referral?
No. Minnesota’s direct access law allows you to schedule an evaluation without a referral. Medicare patients still need one, and we can help arrange that if necessary.
How long is each session?
Initial evaluations are about 60 minutes. Follow-up visits are typically 45 minutes unless more time is needed.
What if my insurance does not cover direct access?
We offer transparent cash rates and flexible payment plans. You can also use HSA or FSA accounts to cover out-of-pocket costs.
Can I switch to maintenance or home exercise only?
Yes. Once you meet your goals, we can transition you to a self-guided home program with periodic check-ins to ensure you stay on track.
