What Situation or Injury Led You to ORSI?
After my diagnosis of breast cancer, I underwent a bilateral mastectomy and lymph node removal. The surgery restored my health but left me with significant challenges in the range of motion of my shoulder, arm swelling, and the early stages of lymphedema. Simple tasks like raising my arm to brush my hair or reaching for a book on a shelf were met with pain and stiffness. I knew I needed specialized rehabilitation to regain my function and quality of life.
How Would You Categorize Your Story?
My journey with OrthoRehab Specialists has been one of restoration and empowerment. From the first evaluation to the latest exercise progression, the team helped me rebuild strength, reduce swelling, and learn techniques that keep my lymphatic system moving. I went from living with daily discomfort to feeling confident in my body again.
Please Share Your Experience
The care at OrthoRehab was exceptional from day one. My physical therapist performed a comprehensive evaluation that included shoulder range of motion testing, measures of limb circumference, and a discussion of my home routine. I was introduced to a series of manual lymphatic drainage techniques, scar mobilization, and targeted exercises to improve shoulder mobility. Each visit I received clear instruction, gentle hands-on treatment, and an updated home-program to practice on my own. The therapists explained the rationale behind every technique and exercise so I could take ownership of my recovery.
What Goals Did OrthoRehab Help You Accomplish?
- Restored Arm Mobility: I went from being unable to lift my arm above shoulder height to regaining full overhead reach within eight weeks of therapy.
- Lymphedema Management: Through manual lymphatic drainage, compression garments, and daily self-care routines, my arm swelling reduced by two inches in circumference and has remained stable.
- Improved Strength: Progressive resistance exercises for my rotator cuff, scapular stabilizers, and core muscles gave me the power to lift light weights and return to gentle yoga.
- Enhanced Confidence: Understanding the mechanics of my shoulder joint and the principles of lymph flow empowered me to return to activities such as gardening, driving, and light housework with minimal discomfort.

Initial Evaluation and Custom Treatment Plan
The first step in our Cancer Rehabilitation program is a thorough assessment:
- Medical History Review: We discuss your diagnosis, surgical procedures, oncologist guidance, and any previous physical therapy experiences.
- Physical Examination: This includes measuring shoulder flexion, abduction, and external rotation, as well as limb circumference at multiple points to gauge swelling.
- Functional Testing: We observe your ability to perform daily tasks such as reaching, dressing, and carrying light objects.
- Goal Setting: Together we define short-term objectives like improving sleep comfort and long-term goals such as returning to recreational activities.
Based on this evaluation, your therapist will design a treatment plan that blends hands-on techniques with home exercises. You can learn more about our Manual Therapy Services and Therapeutic Exercise Programs.
Manual Lymphatic Drainage and Compression
Manual lymphatic drainage (MLD) is a gentle manual technique that encourages movement of lymph fluid out of the affected limb. Our therapists are trained in Certified Lymphedema Therapist protocols and will:
- Perform light, rhythmic strokes toward lymph node basins
- Teach you self-drainage techniques to continue between sessions
- Recommend and fit appropriate compression garments
Compression garments help maintain the gains achieved with MLD by providing external pressure that prevents fluid accumulation. Our clinic partners with suppliers to ensure a proper fit and coverage for insurance.
Scar Tissue Mobilization and Range of Motion
Scar tissue mobilization helps break up adhesions that can limit movement and cause discomfort. Techniques include:
- Gentle micromanipulation of the scar and surrounding fascia
- Instrument-assisted soft tissue mobilization
- Stretching protocols to improve chest wall and shoulder flexibility
Improving scar mobility not only enhances shoulder range but also reduces tightness that can contribute to nerve irritation and discomfort. Check out our Scar Management Techniques article for tips you can perform at home.
Exercise Prescription and Functional Training
Therapeutic exercises are essential for rebuilding strength and stability. Your program may include:
- Gentle pendulum swings and wall crawls
- Resistance band shoulder abduction, internal rotation, and external rotation
- Scapular retraction and depression drills
- Core stabilization exercises such as planks and bridging
- Postural correction and breathing mechanics
Functional training integrates these movements into real-life activities. For instance, practicing lifting a cup, hanging clothes on a drying rack, or reaching overhead simulates daily tasks and builds confidence.
Axillary Web Syndrome / Cording Care
Axillary web syndrome, also known as cording, presents as tight, rope-like structures extending from the armpit down the inner arm. Our approach to cording includes:
- Scar and fascial mobilization
- Gentle stretching into shoulder abduction and external rotation
- Soft tissue release techniques to break up the cords
Early intervention often prevents the cords from becoming restrictive. Learn more about Axillary Web Syndrome Care on our website.
Return to Activity and Long-Term Wellness
Rehabilitation does not end when swelling subsides. We guide you through a progression of:
- Low-impact cardiovascular activities such as walking or recumbent cycling
- Light resistance training with dumbbells or machines
- Balance and proprioception drills to reduce fall risk
- Mind-body techniques like diaphragmatic breathing and gentle yoga stretches
Our goal is to ensure you have a sustainable program that supports your long-term health. You can explore our Wellness Programs for additional group classes and community resources.
